Transition Planning and Post-Secondary Success For high school special education teachers, the transition plan may be the most consequential document they produce. This 45-hour bundle develops comprehensive transition planning skills: IDEA requirements, age-appropriate transition assessment, post-secondary goal writing across all three domains, Section 504 for post-secondary access, and the full landscape of pathways — employment, travel training, college, self-determination, recreation, vocational assessment, and community living.
